As soon as a resident arrives at one of our schemes, they are introduced to their dedicated support worker. Together, they complete the onboarding process, and within 48 hours, a personalised support plan is developed to meet their individual needs. From this, an action plan is created to guide next steps and ongoing support, with a focus on preparing for move-on accommodation.
Each resident works closely with their dedicated support worker to develop a tailored support plan. This outlines individual needs, sets achievable goals, and identifies the right support and services to meet them. The support plan is a living document that empowers residents to take ownership of their progress and build the skills and confidence needed for long-term stability. We take this process seriously, plans are reviewed and updated on a weekly basis to ensure they remain relevant and effective.
Both the support plan and formal progress reviews are carried out weekly to reflect short-term changes, monitor progress, and respond to immediate needs. These structured sessions assess longer-term progress, revisit goals, and ensure the support remains aligned with each resident’s development and aspirations.
Move-on planning is embedded into this review process from an early stage, with clear steps identified toward securing suitable accommodation. Our team will provide practical assistance throughout, including referrals, tenancy readiness, and coordination with housing providers to enable a smooth and sustainable transition.

We believe in providing the right help, at the right time — without delay. That’s why we have our own dedicated Substance Support Worker on-site. This means we can support residents with substance use issues straight away, without having to wait on outside referrals.
What this means for our residents:
We offer weekly one-to-one sessions to help people manage or reduce their drug and alcohol use. Each session is structured, recorded, and personalised to the individual. Support includes setting goals, reducing harm, preventing relapse, and referring to specialist services when needed.
Having a regularly updated Substance Support Plan means we truly understand each person’s needs and how their substance use may be affecting their life and those around them. It helps them see that it's okay to have reached a difficult point, it’s not the end of the road. There is hope, and we are here to offer the right support to help them move forward. That support starts here, in this room.
